The Community Care Forum (CCF) is organised once every two years. The first Forum “Up and Up: A Forum on Community Care” was held in 2013, and was attended by more than 800 foreign and local participants.

For 2015, the Forum’s theme – “A Conversation about Tomorrow’s Care”, will focus on how the Community Care sector can work together to imagine, rethink and develop new ways of providing better, safer and cost-effective models of care for the future.

The main objectives of this Forum are to:

  • Highlight innovative programmes and see how they can be adapted to the local context such as housing and service design, and technology in community care
  • Feature the importance of client and caregiver empowerment, and their involvement in service design and development, de-stigmatisation efforts and campaigns, specifically for dementia, mental health and end-of-life care
  • Showcase concepts and practice of teamwork, multidisciplinary and trans-disciplinary care in community care
  • Introduce social and community concepts, partnerships and services to build an inclusive society which supports ageing-in-place

There will be three Forum tracks which will focus on the following:

  • Community Care
  • Imagine, Dream and Evaluate about tomorrow’s care
  • Caregiver Well-Being and Mental Health

We have invited a panel of renowned experts from overseas and Singapore to share on their area of expertise through plenary lectures and workshops.

We encourage all allied healthcare professionals, physicians, nurses, social care professionals, administrators and community partners in Singapore and around the world to participate in this Forum. You will have ample opportunities to network and to exchange ideas and best practices applicable to your work.
